If you live in Colorado, Ohio or Texas and are in need of mental health therapy or psychological assessment services and wish to work with Dr. MacIntire, please contact her at Lotus Counseling and Psychological Services (https://www.lotus-counseling.com/ or call 469-431-5616 extension 708).

**If you are experiencing a mental health emergency, please call 911 or go to the nearest emergency room.** The phone number for this practice is for scheduling purposes, only, and is NOT for crisis services. This practice is not currently offering mental health services. The person answering the phone may have no medical or mental health training.
For former clients of Dr. MacIntire in Colorado and Ohio:

For Colorado clients/ patients, files are kept for 7 years from the last date of service for adults and 7 years from a minor's 18th birthday in accordance with Colorado laws.
For Ohio clients/ patients, files are kept for 7 years from the last date of service for adults and 2 years from a minor's 18th birthday or for at least 7 years from the last date of service in accordance with Ohio laws.

If you worked with Dr. MacIntire in Grand Junction, CO, as an individual therapy client/ patient, please request your file by calling 469-388-0280. You will need to complete paperwork for HIPAA compliance.

If you had a psychological evaluation (a several hours of testing with a really long report) that was paid for by the Division of Vocational Rehabilitation or Department of Human Services (you didn't pay me directly), you must request those files from the agency who paid for your testing, as that agency paid to have the file created for use with their program. If you have trouble getting the agency to release your report, please call 469-388-0280 and Dr. MacIntire and her team will do their best to help you. ​

For former clients of Action Potential Advantage in Texas:

For Texas clients/ patients, files are kept for 7 years from the last date of service for adults and 5 years from a minor's 18th birthday or for at least 7 years from the last date of service in accordance with Texas laws.

This practice does not currently offer mental health services (i.e., the diagnosis and treatment of mental health disorders). Performance enhancement coaching files are kept, but they are not medical files. Health and Wellness coaching files constitute medical files if you shared information about your current health status or conditions. For current clients or patients wishing to request medical records, ask for a release of information form from your provider. One will be sent to you through the practice portal to protect your privacy.​ If you no longer have access to the practice portal, call the office at 469-388-0280 and discuss your request with Dr. MacIntire's team.

In accordance with Texas House Bill 4224, which is meant to provide information for Texas residents who are mental health clients to request files and make complaints, this notice is provided. While this practice is not currently offering mental health services, per se, the business is owned and operated by a licensed psychologist. No Surprises Law
The No Surprises Act is a federal law that was passed in December 2020 that was meant to curtail surprise bills by large hospitals and help control healthcare costs. Despite the fact that this practice is not directly offering healthcare services, the law requires that all licensed practitioners post a government-issued disclosure. None of the services provided through this practice are medically necessary healthcare services. There is no medical justification for the delivery of performance optimization services. All fees are published on in practice documents or on this website. ​
